We know you have many choices when choosing a Dentist in Essex, CT so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Dr. Rotella is a participating provider with Delta Dental, Anthem and MetLife. We will; however, submit to any insurance company for you and will do our best to help you maximize your insurance benefit. Please bring all up to date insurance information with you to your first appointment and we will take care of the rest!
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
We are committed to providing you with the highest quality of care and our fees are a reflection of that quality care. We offer a variety of financial options to enable you to receive the dental care you need. We accept cash, check, and all major credit cards.
We will communicate all recommended treatment options and associated fees prior to the start of treatment. Payment is expected at the time of treatment. We do offer several payment plan options to assist in managing the finances of your dental treatment. Please inquire if you are interested in one of these options.
We are committed to respecting your time and ask that you make every effort to keep the appointment time reserved exclusively for you. Should you find it necessary to reschedule an appointment, please provide us with a 24 hour notice to avoid being charged the missed appointment fee of $45.00.
As a courtesy to our patients utilizing dental insurance, we will submit claims on your behalf. We require that any applicable deductibles and estimated patient portion be paid at the time treatment is rendered. We are a participating provider with Anthem, Delta and MetLife, but will submit to all insurance companies
Important Facts About Your Dental Insurance
- Dental insurance is a contract between the patient and the insurance company. It is a benefit to assist you with the cost of dental care. At no time should insurance benefits compromise your doctor’s diagnosis or affect your choice of treatment.
- It is your responsibility to understand the details of your insurance plan and the specific benefits selected by you and/or your employer (e.g. % coverage, deductibles, frequency limitations and annual maximum).
- You (not the insurance company) are responsible for the fees of services rendered.
